Short answer: freshly expressed breast milk keeps for up to 4 hours at room temperature (25°C or cooler), up to 24 hours in an insulated cooler bag with frozen ice packs, up to 4 days in the fridge and about 6 months in the freezer (up to 12 months is acceptable). In Indian summers, when rooms are well above 25°C, don’t rely on room temperature; chill milk straight away.

These timings follow guidance from the US Centers for Disease Control and Prevention (CDC), which is widely used by paediatricians in India too. Always follow your own doctor’s or lactation consultant’s advice if it differs.

Why Indian heat changes the rules

The “4 hours at room temperature” guideline assumes a room at 25°C or below. From April to September, most Indian homes, offices and cars are much hotter. In those conditions, treat room temperature as unsafe for storage and put milk into the fridge or a cooler bag with ice packs as soon as you’ve expressed it. During power cuts, keep the fridge door closed and move milk to a cooler with frozen packs if the outage is long.

How to store breast milk at work

  1. Pump into clean containers. Use breast milk storage bags or clean, BPA-free bottles with tight lids. Wash your hands first.
  2. Label each container with the date and time.
  3. Chill immediately. If there’s an office fridge, store milk at the back, not in the door. If not, use an insulated breast milk cooler bag with ice packs, which keeps milk safe for up to 24 hours.
  4. Carry it home cold. Keep milk in the cooler with frozen ice packs during the commute, then move it straight to your fridge or freezer.
  5. Store small amounts. 60–120 ml per container reduces waste, because thawed milk can’t be refrozen.

In India, the Maternity Benefit (Amendment) Act, 2017 requires establishments with 50 or more employees to provide crèche facilities, which many offices pair with a nursing room. It’s worth asking HR what space is available for pumping and storage.

Travelling with breast milk

  • For day trips and train journeys, a cooler bag with 2 ice packs is usually enough.
  • Freeze ice packs overnight so they’re fully solid when you leave.
  • For flights, check the airline’s and airport’s current rules on carrying expressed milk and ice packs in hand luggage, and keep milk easy to show at security.
  • A dedicated breast pump bag with an insulated compartment keeps the pump, parts and milk together.

How to thaw and warm breast milk

  • Thaw overnight in the fridge, or hold the sealed bag under running lukewarm water.
  • Warm by standing the bottle in a bowl of warm water. Never microwave breast milk; it heats unevenly and can create hot spots that burn a baby’s mouth.
  • Swirl gently to mix the fat that separates. Separation is normal.
  • Test a few drops on your wrist: it should feel warm, not hot.
  • Use thawed milk within 24 hours if kept in the fridge, or within 1–2 hours once at room temperature. Never refreeze.

Does breast milk smell or look different after storage?

Stored milk may look bluish, yellowish or creamy, and the fat rises to the top. Some mothers’ milk develops a soapy smell after freezing because of an enzyme called lipase. Most babies still drink it, and it’s considered safe. Milk that smells clearly sour or rancid should be thrown away.

Frequently asked questions

Can I mix fresh breast milk with refrigerated milk?

Yes, but cool the fresh milk in the fridge first, then combine it with milk expressed on the same day.

Can I put breast milk back in the fridge after the baby drinks some?

No. Once a baby has fed from a bottle, use the rest within 2 hours and then discard it, because bacteria from the baby’s mouth get into the milk.

How long can breast milk stay in a cooler bag?

Up to 24 hours with frozen ice packs, according to CDC guidance. Then refrigerate, freeze or use it.
